Medication Form It has come to our attention that children have been coming to school with prescription medications , Advil, inhalers etc. in their back packs. All medications are to be kept in the office with parental instructions attached to them No student will be allowed to carry any medications (antibiotics or over the counter medications) on them during school hours. Where a student requires a specific medication for protection of life, the medication may be carried by the student on his/her person as long as staff is aware. The attached form may be used for prescription requests and over the counter requests. Please note on the bottom of the form we ask that you identify your child if they carry an inhaler on them to school (Physician’s Signature required).
It is the responsibility of parents/guardians to ensure that any and all special requirements or methods of application are fully explained to school staff. Your child is our number one priority. Thank you for your attention in this matter.
